Consent and Verification Process

Since the Coronavirus outbreak we are adapting how we work and reducing the number of people coming into see us. This allows us to support those who need urgent or emergency care and reduce the risk of infection.

It is likely that this situation will continue for the foreseeable future, so in line with national guidance, it may be some time before our clinics are operating fully.  

Consent

So, to help us continue to provide a safe and effective service now and plan for the future, we’d like to:

  • Check your details to ensure we have the most up to date contact information for you as a patient on our waiting list  
  • ask for your consent (please complete - Consent Form DynamicHealth [Word]) to be able to contact you in new ways (e.g. by email instead of post) and return to s1.dynamichealth@nhs.net

Verification

Once you have received and returned your consent form, you will be added to out waiting list. Then you will receive an email that contains a link, which will take you to a secure website  to verify your details. it's important that you do so within 7 days of receiving this email.
